What this site collects, where it goes, and what we never do with it.
This policy explains what happens to information you send us through this website. It sits alongside our Notice of Privacy Practices, which governs health information we hold as your treatment provider.
What our forms collect
Our “Request a Visit” forms collect your name, phone number, email address, and — optionally — the type and description of your wound and your insurance. Our facesheet page lets a facility or agency send us a patient’s referral document. Information of this kind is protected health information, and we treat it that way from the moment you press submit.
Where it goes
- Submissions are transmitted over an encrypted connection (TLS) directly to our own clinical system. We do not use a third-party form processor.
- This website runs no analytics, no advertising pixels, and no tracking cookies. We deliberately keep them off pages where you might tell us about your health.
- The only request this site makes to an outside company is to Google Fonts, which loads typefaces and receives your IP address. It receives no health information.
- Access to your submission inside our system is limited to authorized staff and every view is written to an audit log.
How we use it
To respond to your request, verify your insurance benefits, schedule care, and coordinate treatment with your physician and facility. We do not sell your information, and we do not share it for advertising.
Calls and texts
When you give us your phone number, you are asking us to get back to you — we may call or text you about your request and your care. Message and data rates may apply. Reply STOP to any text to opt out, or HELP for help. Consenting to calls or texts is never a condition of receiving care.
Retention and your choices
If you become a patient, your information is retained as part of your medical record for the period required by Illinois and federal law. If you do not, we keep your inquiry only as long as needed for follow-up and our records. You may ask us what we hold about you, or ask us to correct or delete an inquiry, by calling (773) 389-7743. Patient rights to access and amend a medical record are described in our Notice of Privacy Practices.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them through it.
